Why exactly is healthcare and insurance more expensive than what (we think) they should be?

So much of what Obama says is the cost to remedy the "skyrocketing" premiums, etc. I have never heard him say how these problems are first and foremost. Why is there not enough to run? There is absolutely no shortage of people want to be doctors. Why not just accept the fact that more students in medical and nursing schools?
